How to Get Started

Contact Kinship House at (503) 460-2796 ext. 106 and speak with our Intake Coordinator.

Both outpatient and assessment first time appointments are generally scheduled two weeks after all paper work is completed and received.

Kinship House accepts many insurances including OHP and private payment. Child Welfare Assessments are usually funded through the Department of Human Services.

Outpatient Therapy Referrals

Guardians, parents or caseworkers who are interested in outpatient mental health therapy need to complete our Intake forms. Please click on this link to fill out our INTAKE PACKET If you have any questions, please contact our Intake Coordinator at (503) 460-2796 ext. 106.

Permanency and Needs Assessment Referrals (Best Interest)

The assessment process takes 90 days to complete. This process begins once the referral form is submitted online.   Prior to the assessment being assigned to a clinician, the following information must be submitted via email, fax or delivered in person:

  • Release of Information for the foster parent, therapist, daycare provider, CASA and/or teacher
  • Initial Assessment
  • Child Welfare Case Plan
  • CARES Evaluation
  • IEP Report/Evaluation
  • Psychological Evaluation
  • Early Intervention Report

Once the above information and the appropriate Medical Administrative voucher (please refer to the last page of the referral paperwork for assessment codes) have been received, a clinician will be assigned. Each clinician has their own schedule so the Intake Coordinator is unable to schedule assessments.

From time to time, we may need to close referrals due to being at capacity and will provide this information on our website.
